ABSTRACT

<p><b>OBJECTIVE</b>By the detection of hepatocellular carcinoma in patients with GP73 content to explore its significance for liver cancer diagnosis and clinical treatment.</p><p><b>METHODS</b>Up-converting phosphor immunochromatography methods was used for detection of serum GP73 in 302 cases of liver cancer, chronic hepatitis, cirrhosis patients and normal control, application electrochemiluminescence detection of AFP as a comparison. And ROC curve analysis of its diagnostic performance, and patients with HCC before and after treatment GP73 periodic testing of the treatment effect.</p><p><b>RESULTS</b>GP73 content of liver cancer patients was (160.83 +/- 37.24) ng/ml, significantly higher than the chronic hepatitis, cirrhosis patients and normal controls. The area under ROC curve for GP73 diagonosis of hepatocellular carcinoma and chronic hepatitis and cirrhosis was 0. 907 which higher than the area under the ROC curve of AFP was 0. 727. Determine 110 ng/ml most of its critical value, the sensitivity was 84.7%, specificity was 75.8%; liver cancer patients with AFP < 400 ng/ml, detection rate of GP73 was 71.15%; the GP73 content after treatment showing an overall downward trend.</p><p><b>CONCLUSION</b>GP73 has a high sensitivity for the diagnosis of HCC can add to the AFP's lack of sensitivity that can be used to monitor liver cancer treatment.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the clinical significance of hepatitis B virus large envelope protein (HBV-LP) in diagnosing viral replication, we detected the HBV-LP and HBV DNA in the serum of the patients infected with HBV.</p><p><b>METHODS</b>Serum HBV DNA was detected by fluorescent quantitative PCR (FQ-PCR). HBV-LP was analyzed by enzyme linked immunosorbent assay (ELISA).</p><p><b>RESULTS</b>(1) No significant difference between the detectable rate of HBV DNA and HBV-LP was found in the HBeAg negative patients without of anti-virus treatment. Of all HBeAg negative patients without of anti-virus treatment, the copies of HBV-DNA was related positively to the absorbency HBV-LP. (2) There was significant diference between the detectable rate of HBV DNA and HBV-LP in the HBeAg negative patients after anti-virus treatment.</p><p><b>CONCLUSION</b>There is a perfect correlation between the positive rate of HBV-LP and HBV DNA in the HBeAg negative patients.The HBV DNA is a reliable serological marker that can reflect the replication of HBV in the HBeAg negative patients.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To document the indication and value of orthotopic liver transplantation (OLT) for the patient with Klatskin tumor.</p><p><b>METHODS</b>Forty patients of Klatskin tumors, including 5 patients with liver transplantation (LTx) and 35 without LTx (WLTx) from Jan 1992 to Dec 2003 were enrolled for analysis retrospectively. The TNM stages were comparable in both groups. In LTx group, 4 patients were Klatskin tumor including 1 recurrence 5 months later after resection, and 1 cancerization from Caroli's disease. Biliary duct anastomosis was performed by Roux-en-Y choledochojejunostomy in two, end-to-end choledochocholedochostomy in 3 cases.</p><p><b>RESULTS</b>In LTx group, the total resection rate and radical resection rate were both 100%. Four cases survived for 48, 38, 21 and 5 months, respectively, except that the other one died from bile leakage at the 40th day after transplantation. All 4 survivors had good life quality without tumor in local or distant site, even though 3 of 4 cases developed biliary stricture, which was cured by radiological intervention therapy. The overall 1, 3-year survival rates were both 4/5 in LTx. The total resection rate and radical resection rate in the WLTx group were 63.0% (17/27) and 40.7% (11/27) and, the 1, 3, 5-year survival rates were 32.2%, 8.0% and 0, respectively. There was significant difference between the two groups in radical resection rate and survival rate (P = 0.016). In the routine radical resection group, the 1, 3-year survival rates were 54.5% (6/11) and 18.% (2/11), which were not significantly different from those in LTx.</p><p><b>CONCLUSION</b>OLT is a good choice for the patients with advanced stage of unresectable Klatskin tumor by routine modalities, and the prognosis was exciting.</p>
